The general process
- Gather legal proof of the name change (marriage certificate, divorce decree, or court order).
- Notify your board of nursing and submit its name-change form with the documentation.
- Update your government-issued ID so it matches.
- Confirm the updated name appears on Nursys.
What varies by state
Accepted documents, forms, fees, and whether a new license card is issued vary by board and change. Confirm with your board of nursing — we do not publish state-specific requirements. If you also changed your legal residence, review your primary state of residence, which affects a multistate license.
